Abstract

Aims: Investigate the Voice Handicap Index (VHI), the Quality of Life (QoL) and the correlation between VHI and QoL in patients with T1-T3 laryngeal carcinoma treated with radiotherapy or laser surgery. Other aims were to study the effect of tumour location, TNM-stage and treatment on VHI and QoL. Methods: Thirty-seven patients with T1-T3 laryngeal carcinomas, treated with radiotherapy or laser surgery, participated in this study. Subjective voice quality and QoL were measured before and after treatment. Evaluation of voice handicap was done with the VHI. QoL was evaluated with The European Organization of Research and Treatment of Cancer Quality of Life Questionnaires (EORTC QLQ-C30 and EORTC QLQH& N35). Results: Patients showed a statistical significant increase in QoL 6 months after treatment. Patients showed a better Global Health/QoL and a decrease in symptomatology. VHI correlated significantly with Global Health/QoL, the Emotional and Social functioning scales, insomnia, pain, swallowing, sense- teeth- and speech problems, trouble with social contact, less sexuality, dry mouth, sticky saliva and coughing. Meaning that with better voice quality patients had less symptoms and better social and emotional functioning and a better overall QoL. Conclusion: This study shows that both the EORTC C30 and the EORTC H&N35 showed correlations with the VHI total score. Future studies regarding to voice quality and overall QoL in laryngeal cancer patients should combine these questionnaires to get a better view of the influence of voice quality on the QoL of patients.
