Abstract

Epilepsy is a condition in which seizures arise, caused by an imbalance between cortical excitation and inhibition. The aim of this study was using transcranial magnetic stimulation combined with electroencephalogram (TMS-EEG) to observe differences in cortical excitability between epileptic patients and healthy subjects. We studied 14 epileptic patients (4 men, mean age 25 years) en 18 healthy subjects (11 men, mean age 28 years) and stimulated the motor cortex and occipital cortex, both left and right. On each site 75 stimuli were given with an intensity of 110% of motor threshold (MT). The EEG signal of one channel at each stimulation position is averaged and the amplitudes and latencies of the peaks were defined. We observed that the MT of epileptic patients is higher than of healthy subjects, when the patient without anti-epileptic drugs (AED) is taken out of the analysis, this difference is significant. When the motor cortex is stimulated, the amplitude is increased in epileptic patients, suggesting a higher excitability, but this difference is not significant. When the occipital cortex is stimulated, the amplitudes of the two latest peaks are significantly decreased in epilepsy. Also, we saw influences of the AEDs on the MT and amplitude. We found that with TMS-EEG it is possible to find differences in cortical excitability between epileptic patients and healthy subjects. TMS-EEG could be used to improve the epilepsy diagnostics.
