Abstract

Introduction: Prominent ears are the most common congenital ear deformity in children. Despite the fact that prominent ears do not have any functional consequences, it can have a psychological impact and affect self-esteem for children and adults. The aim of this study is to compare cartilage-sparing and cartilage-cutting techniques in otoplasty, with emphasis on complications and best post-operative results. Methods: A retrospective observational database study was conducted in the Medisch Centrum Leeuwarden (MCL) hospital for the period 2002-2012 (11 years). Patients included in the study received otoplasty using the following techniques: Mustardé (11%), Chongchet (65%), Furnas (1%), conchashell reduction (13%) or a combination of these techniques (10%). All data analysis was performed by Chi 2 tests, T-test, Fischer exact, Anova and logistic regression analysis with in the program SPSS (IBM) Results: 565 patients (1060 ears) were included in the study. 216 ears (with a maximum of 238 ears) developed a complication, with 6% requiring revision of the otoplasty. Within specific techniques Mustardé has a 20% complication rate where 7% needed a revision. Chongchet 22% with a revision rate of 6%, Furnas at 17% and 0%, lastly the Conchashell reduction with a 17% complication rate and a 7% revision of the otoplasty. There was no difference between the incidences of complication in cartilage-sparing (20%) or cartilage-cutting (21%) techniques for otoplasty (p =0.44). The aesthetic research shows cartilage cutting techniques score significantly lower statistically with postoperative auricular deformity (p =0.44), overall appearance (p =0.03) and satisfaction (p =0.04) compared to cartilage-sparing technique. Conclusion: The study shows that there was no significant difference between the groups of patients with complications in comparison to the patients with no complications. Patients who have received operations with cartilage-sparing or cartilage-cutting techniques are at the same level of risk for developing a complication. The aesthetic research showed a higher postoperative auricular deformity when using the (cartilage-cutting) Chongchet technique compared to the (cartilage-sparing) Mustardé technique. With regards to overall appearance and satisfaction, Mustardé scored significantly better than the (cartilage-cutting) conchashell reduction technique.
