Abstract

INTRODUCTION: Patients on hemodialysis as renal replacement therapy are frequently suffering from symptoms and complications of therapy. This is mainly caused by the inaccurate measurement of the hydration status with the current techniques. Dehydration in hemodialysis patients can cause progressive loss of the remaining kidney function and causes symptoms like hypotension, muscle cramps, thirst, headache, nausea, dizziness and other neurological signs. Overhydration in hemodialysis patients produces an extra volume load on the cardiovascular system and causes symptoms like dyspnea and edema. Because of this, the comorbidity and mortality are significantly increased in hemodialysis patients. The aim of this study was to examine whether weekly chances in the bio-impedance parameter overhydration, measured by body composition monitor (BCM), could predict signs and symptoms related to dehydration or overhydration. MATERIAL AND METHODS: The research population in this study was formed by patients whom had their hemodialysis treatment at Dialyse Kliniek Noord at Beilen. All patients were screened for exclusion criteria, before they were approached for participation. Medical history was taken before hemodialysis. Information on signs or symptoms where scored on observation lists for 10-12 weeks. During this study the bio-impedance measurements were obtained by a calibrated BCM machine. This machine measures bio-impedance according to the bio-impedance spectroscopy (BIS) method. All bio-impedance measurements were taken before the mid-weekly dialysis session. RESULTS: In total, 16 patients gave their informed consent for participation in this study. Three of them died, and one of them went for treatment to another dialysis clinic. Of the remaining 12 patients, data could be supplemented for 9 patients from a previously performed study. There was no difference in the delta BCM overhydration parameter between the group without signs and symptoms in comparison to the group whom experienced signs or symptoms related to dehydration (p=0.28). There was a significant difference in the delta BCM overhydration parameter between the group without complaints in comparison to the group whom suffered from signs and symptoms related to overhydration (p<0,001). Against all odds was the average of delta BCM overhydration parameter lower in the group whom had signs and symptoms related to overhydration in comparison to the group without signs and symptoms. CONCLUSION: We must conclude that weekly measured delta BCM overhydration parameters cannot predict signs or symptoms related to dehydration or overhydration for chronic intermittent hemodialysis patients. To test whether the same is true for the individual chronic intermittent hemodialysis patient, data of included patients should be supplemented.
