Abstract

Objective: The purpose of this study was to evaluate the influence of the crown - implant ratio on the survivalrate of a prospective study for short implants (8.5 mm). And to compare the crown- implant ratios measured with digital dental casts and digital intraoral radiographs. Setting: Department of Oral and Maxillofacial Surgery, University Medical Center Groningen, Netherlands Design: This is a pilotstudy of a prospective study in short implants (8.5 mm). Methods: 44 patients and 76 implants were selected using inclusion criteria for this study. The patients had implants in the posterior section of the maxilla and/or mandibula. The implants and the implant crowns were in function for one year. Dental plaster casts with implant analogue and digital intraoral radiographs of the implant section were collected for this group. The dental plaster casts were scanned with a 3Shape- Scanner (Zeno Wieland I 1-Mes, Pforzheim, Germany). The occlusion point of the implant crown with the antagonist was collected intraorally. With this occlusion point and de digital scan the crown length was measured. With a digital program (Dicomworks: Joop de Vries©) the crown length on the digital intraoral radiographs was measured. The implant length was 8.5 mm. Then the crown - implant ratios were measured with both methods. Data were analysed using a Bland and Altman plot. The influence of the crown- implant ratio measured with both methods was evaluated. Results: The mean crown- implant ratio measured with digital scans was 1.14 ± 0.19 and with digital intraoral radiographs 1.27 ± 0.21. The Bland and Altman plot shows a strong agreement between the two measure methods with 95% confidence interval. From 76 measurments 72 (94. 7%) were in the limit lines of the plot. The survival rate of the short implants (8,5 mm) and the implant crowns is 100%. There is no influence of both measuring method on the survival rate in this study. Conclusion: This study suggest that there is a strong agreement in the measurement of the crown - implant ratio with digital scans and digital intraoral radiographs. Other consideration has to be made to choose a measuring method. A combination of both methods will give probably an optimal view of the crown - implant ratio. The influence of the crown - implant ratio could not be calculated between both methods and the survival rate of the implants of a prospective study for short implants because of the 100% survival rate.
