Abstract

Traditional Chinese Medicine (TCM) is practiced in China for over 2000 years and its popularity in Western countries has increased over the years. TCM has various diagnostic methods, one of these is tongue inspection. TCM assumes that sections of the tongue correspond to an organ in the body. Discoloration or changes in a particular section of the tongue may indicate a problem with its associated organ. The aim of this study is to assess the validity and specificity of tongue diagnosis and determine if tongue diagnosis has an added value for the diagnosis of diseases. The study is attempts to answer three main research questions: I. What are the criteria to apply TCM? II. Does diagnosis with TCM show comparable results as diagnosis with Western methods? III. What kind of diseases can be diagnosed with tongue inspection? The database of PubMed, the Cochrane Library, Web of Science and SciVerse were consulted to find suitable articles for this systematic review. 18 of the 34 articles that satisfied the inclusion criteria, were selected after quality assessment. The results are presented in tabular form and subsequently elaborated per research question. There results showed that there is consensus about what a healthy tongue is supposed to look like: light red with a white coating. Assessment of the tongue can be performed by a TCM evaluator. To limit the variation between TCM evaluators a digital analysis system can also be used. Diagnosis is often made through the Western method, and then compared with TCM results. The main conclusions that can be drawn from this study are: I. First of all the tongue can be divided into a number of segments, secondly the tongue can be assessed on the color of the tongue, the color of the tongue coating, teeth marks, consistency of the tongue coating, sublingual blood vessels, tongue movement and tongue fissures. II. Mainly the changes in the gastrointestinal area are noticeable on the tongue. III. There are no studies in which first a diagnosis is made by applying TCM. TCM can however be used to confirm a diagnosis obtained with a Western method.
