Abstract

Background: Hidradenitis suppurativa (HS) is a chronic inflammatory, debilitating follicular skin disease with painful nodules, abscess formation and, in later stages, scarring, predominantly in the inverse areas. The disease is often difficult to treat and patients experience a decreased quality of life. Limited data is available on factors influencing disease severity. Early detection of patients at risk for severe HS is important to set early and intense treatment. Objective: To identify factors that are associated with the severity of HS. Methods: 770 HS patients treated in two outpatient clinics with special interest in HS from the Netherlands were included in this study. HS severity was assessed using the Hurley classification system and patient characteristics were recorded with a standardized form during the first consults. Results: Hurley III patients were more commonly males (p <0.001, OR 2.7), had more frequently affected armpits (p<0.001, OR 4.2) and perineal/perianal areas (p<0.01, OR 2.4), and reported a higher co-occurrence of DM (p<0.01, OR 3.3). Hurley I patients were more commonly females (p 0.04, OR 1.5) and had less frequently affected armpits (p<0.001, OR 0.38) and mammary areas (p 0.03, OR 0.64). Conclusion: This study shows that disease severity of HS is associated with the male gender, involvement of the armpits, perineal/perianal and mammary areas and an increased co-occurrence of DM. Long-term prospective studies are necessary to confirm the prognostic role of these factors.
