Abstract

Background: Blood pressure measurements play an essential role in antenatal care, but the current standard method, sphygmomanometry, is thought to be unreliable. Recently, operator-independent, automated oscillometric blood pressure devices have become available and are increasingly being used. However, most of these devices are not validated, especially not in pregnancy. The aim of this study was to compare four different techniques to measure blood pressure in pregnancy. Methods: Blood pressure measurements were performed in 200 pregnant participants using sphygmomanometry and three oscillometric devices (Microlife VSA, Uscom BP+ and Tensiomed Arteriograph). Results of peripheral blood pressure were compared between devices. Central blood pressure and augmentation index measured by Uscom BP+ and Tensiomed Arteriograph were also compared. Intra- and inter-device reliability between repeated measurements of the four devices was assessed with Intraclass Correlation Coefficients (ICCs). Bland-Altman analysis was used to compare the peripheral, central blood pressure and heart rate. Subsequently, a two-way mixed-effects model was used to assess the consistency of agreement. Pearson’s correlations were used to compare augmentation index. Results: Intra- and inter-device ICC’s showed excellent agreement for all variables and devices. Bland-Altman analysis showed higher peripheral systolic blood pressures measured with Tensiomed Arteriograph [mean difference 8.02 (Tensiomed Arteriograph - sphygmomanometry) and 6.11 (Tensiomed Arteriograph - Uscom BP+)]. Additionally, Microlife VSA measured higher peripheral diastolic blood pressures [mean difference -5.96 (Tensiomed Arteriograph – Microlife VSA) and -4.97 (Uscom BP+ - Microlife VSA)]. Excellent agreement between the devices was found for heart rate, central systolic blood pressure and augmentation index. Conclusion: Excellent agreement between sphygmomanometry, Microlife VSA and Uscom BP+ was shown in this study. The level of agreement between these three devices and Tensiomed Arteriograph was below standard.
