Background: Early identification of patients at risk of clinical deterioration during an acute exacerbation of chronic obstructive pulmonary disease (AECOPD) is clinically relevant. This study evaluated the association between diaphragm excursion ratio and clinical outcomes in patients admitted with AECOPD. Methods: In this prospective observational cohort study, diaphragm excursion ratio was measured within 48 hours of admission in patients with AECOPD. The primary outcome was clinical deterioration, defined as non-invasive ventilation, intensive care unit admission, or inhospital mortality. Secondary outcomes included hospital length of stay, 30-day COPDrelated readmission, and associations with clinical parameters, including the Modified Borg Dyspnoea Scale. Results: 44 patients were included, of whom 2 (4.5%) clinically deteriorated. Due to the low event rate, statistical analysis of the primary outcome was not feasible. Patients with clinical deterioration showed higher diaphragm excursion ratios than those without (0.91 [0.90–0.92] vs 0.53 ± 0.26). Diaphragm excursion ratio showed a weak correlation with time to medical readiness for discharge (Spearman’s rho = 0.329; 95% CI 0.027–0.576; p = 0.029) and was associated with prolonged hospital stay (>5 days; OR per 0.1 increase 1.52; 95% CI 1.03– 2.23; p = 0.036). Higher Borg scores at discharge were associated with 30-day COPD-related readmission (OR 3.8; 95% CI 1.07–13.6; p = 0.040). Conclusions: Diaphragm excursion ratio is potentially associated with clinical outcomes in patients with AECOPD and may contribute to early risk stratification. Validation in larger cohorts with a higher number of events is required to confirm these findings
