Background Argipressin is recommended for patients with a distributive shock and hypotension that is resistant to catecholamines, at an infusion rate of 0.75 mL/h, and a bolus administration is not permitted. Syringe pumps deliver unreliably at infusion rates lower than 5 mL/h and certain nursing practices could negatively influence this. Research question How quickly does argipressin reach the patient and which parameters influence this speed? To what extent do ICU nurses vary in their practices when starting argipressin? Methods In-vitro measurements were made on an infusion set-up to measure the time till administration of argipressin according to different nursing practices objectified with an online survey. Results At infusion rate 0.75 mL/h, the median time until the first drop of argipressin has reached the patient is 23.80 min (17.15-30.20). The time till administration is shortened by increasing the infusion rate, placing the syringe pump above the patient, using 3-way stopcocks and purging the infusion system. There is variation in standard materials and nursing practices within and between centres. Conclusion There is a preventable delay time till administration of argipressin. Medical protocols lack universal implementability and take the delay insufficiently into consideration. Dilution of medication and increasing infusion rates can overcome the unreliability of low infusion rates, as well as optimally calibrated materials and safe purging.
