Background: Chronic pelvic pain, often localized around the sacroiliac joints, is a disabeling condition that significantly impacts quality of life. Conventional therapies such as physiotherapy and pain medication frequently offer insufficient relief. Prolotherapy, a regenerative injection therapy using hypertonic glucose, is increasingly used internationally for persistent musculoskeletal pain, yet clinical research in the Netherlands is lacking. Objective: This prospective observational study evaluates the effect of prolotherapy on pain reduction and functional improvement in patients with chronic pelvic pain over a period of six months. Methods: Sixteen adults with chronic, therapy-resistant pelvic pain were included in a primary care setting. Participants underwent four prolotherapy treatments, each consisting of six glucose injections around the sacroiliac joint. Outcomes were assessed at baseline and after 20 weeks using the Numeric Pain Rating Scale, Quebec Back Pain Disability Scale, and Active Straight Leg Raise Test. Analyses used paired-samples t-tests or Wilcoxon tests, with effect sizes calculated using Cohen’s d. Results: Prolotherapy resulted in a significant reduction in pain scores (NPRS: -21.9 points, p = .013) and improvement in functional status (QBPDS total score: -10.75 points, p = .025). Significant improvement on the ASLR test was observed on the right side. The largest functional gains within the QBPDS were seen in lifting a heavy suitcase, running 100 meters and turning in bed. Conclusion: Prolotherapy appears to be a promising and safe treatment option for patients with chronic pelvic pain unresponsive to conventional therapies, leading to clinically relevant pain reduction and functional improvement. Larger-scale studies are needed to confirm these findings and support implementation into clinical practice.
