Background - Food allergies are increasing worldwide and place a substantial burden on patients and healthcare systems. Oral immunotherapy (OIT) is a promising treatment, but long-term outcomes remain largely unexplored. This study aimed to evaluate the durability of OIT in young children. Methods - This prospective, exploratory follow-up of the ORKA study included 23 children. Of the 122 original participants, 101 were approached for follow-up. Children who could consume a high allergen dose at home without symptoms were classified as a low-risk subgroup and invited for a home-based food challenge. Following a 12-week elimination period, participants underwent a food challenge test according to the PRACTALL protocol. The primary outcome was sustained unresponsiveness. Secondary outcomes included changes in allergen-specific IgE and potential associations with clinical and demographic characteristics. Results - After elimination, 22 of 23 participants (95.7%) had a negative home challenge, indicating sustained unresponsiveness. One child experienced an allergic reaction to accidental raw egg ingestion and was classified as having lost tolerance. Complete IgE data were available for 13 children. Eight off them showed a decrease and five an increase compared with baseline. Due to the near-uniform outcome and small sample size, no significant associations with clinical or demographic characteristics could be established. Conclusion - OIT in a carefully selected low-risk subgroup of young children can result in sustained unresponsiveness. While further studies are required, these findings represent an important step toward durable treatment options for pediatric food allergy.
