Aims: Finding the cause for insufficient subjective improvement after surgical treatment of stress urine incontinence (SUI) and prolapse. Methods: This observational cohort study at Isala analyzed the results of all surgical procedures of a SUI- and a prolapse group performed between January 1st 2019 and April 29th 2024. Patient Global Impression of Improvement (PGI-I)-, Pelvic Floor Disability Index-20-, treatment goals-, Sandvik Incontinence Severity Index- and Patient Global Impression of severity-questionnaires were administered and compared pre- and 3 months postoperative (along with age, BMI and parity). Both groups were divided into a group of satisfied patients (PGI-I score 1-2) and a group of dissatisfied (PGI-I score 3-7) patients. Causes for insufficient subjective improvement, complications and reoperations were investigated. Results: 571 SUI surgeries and 1662 prolapse surgeries were included with a 3-month followup. The SUI-group showed 81.4% of patients experienced sufficient subjective improvement. Incorrect counseling and/or expectations showed to be the main cause for insufficient subjective improvement. Age >60 and BMI >30 have shown a negative effect on subjective improvement. 22 re-operations were performed, 4 because of a complication. A total of 82 operations showed 123 complications, of which pain (32 cases) and infection (31 cases) were most common. The prolapse group showed 86.7% of patients experienced sufficient subjective improvement. Incorrect counseling and/or unrealistic expectations showed to be the main cause for insufficient subjective improvement. 56 re-operations were performed, 5 because of a complication. 479 operations resulted in 863 complications, of which catheterization (235 cases) and infection (226 cases) were most common. Conclusion: Incorrect counseling and/or unrealistic expectations are the biggest cause for insufficient subjective improvement after surgical treat treatment of SUI and prolapse.
