Introduction: A treatment option for prostate cancer is robot-assisted laparoscopic prostatectomy (RALP), which requires a minimum life expectancy of 10 years. Urinary incontinence is a debilitating complication that can reduce quality of life. According to the current know literature with the conventional RALP method, older patients have a higher risk of incontinence. This retrospective cohort study investigates the relationship between age and incontinence after RALP using the modified maximal urethral length technique (MMULT) developed at Ziekenhuis Groep Twente (ZGT). Methods and Materials: The study included 929 patients who underwent RALP at ZGT between 2017 and 2023. Patients were divided into three age groups: <70 years, 70-75 years, and ≥75 years. Incontinence was assessed at four postoperative time points: six weeks, three months, six months, and twelve months after surgery. The severity of incontinence was measured using the number of incontinence pads (PADs), and quality of life was evaluated with patient-reported outcome measures (PROMs). Results: A significant association between age and incontinence was observed at six weeks and three months postoperatively, with older patients having a higher risk of incontinence. However, this association was no longer observed at six and twelve months, suggesting that MMULT eventually equalizes continence levels across all age groups, although with a delayed effect. PROMs analysis indicated that incontinence was associated with a lower quality of life. Conclusion: The study concludes that RALP with MMULT provides an equal chance of continence among patients of all ages, although the recovery of continence takes longer in older patients. For patients with prostate cancer and a minimum life expectancy of 10 years, RALP with MMULT could be consxidered regardless of age.
