Objective: To investigate the effects of starting Flash Glucose Monitoring (FGM) in daily life on glucose regulation in patients with Type 2 Diabetes Mellitus (T2DM) on intensive insulin regimens. Research design and methods: This study is a retrospective analysis using a within subject design, performed on data from the Diabetes and Lifestyle Cohort Twente. Participants underwent a two-week blinded FGM period and later opted to use FGM in their daily life. Raw glucose data collected by the FreeStyle Libre sensor were used to calculate sensor metrics - including Time in Range (TIR), Time Below Range (TBR), Time Above Range (TAR), mean glucose, and the Glucose Management Indicator (GMI) - for the blinded period, the first 24 hours, the first two weeks and per quarter throughout the first year of FGM use. HbA1c data were also analysed and compared. Results: From the entire cohort, 28 participants had accessible and sufficient sensor data, and 27 of those had adequate HbA1c data. Compared to the blinded FGM period, TBR significantly decreased by 3.57% in Q1 and averaged 2.5%. This effect was sustained in Q2, Q3 and Q4, while the TIR remained unchanged. However, compared to the first day of FGM use in daily life, mean glucose and GMI significantly increased in Q1 by 0.58 mmol/L and 2.52 mmol/mol, respectively, and this effect persisted until Q3. TIR significantly decreased in Q3 in the unblinded comparisons. FGM usage did not result in significant changes in HbA1c levels. Conclusions: FGM usage led to a significant reduction in TBR (p = 0.01) over the first year of use in daily life. However, further research is needed to determine if the results are generalisable to a broader population.
