Abstract

Introduction: Otitis Media with Effusion ( is a common pediatric condition in which fluid accumulates in the middle ear, resulting in conductive hea ring loss as the cardinal symptom. Tympanostomy Tubes ( are a treatment option, if hearing loss of >20dB is found. The Dutch guideline re commends both pre and postoperative evaluation of hearing ( ‘Zorginstituut Nederland’ ( a Dutch Governmental Health care Institute reports that the role of audiometry in treating OME patients with TT needs to be re evaluated, to enhance cost effective treatment. This requires insight into the application of audiometry in current practice. Method: This multi centre retrospective cohort study used data from the Dutch Otologic Quality Registry ( to include patients from tw o hospitals (Deventer Hospital and Diakonessenhuis Utrecht). Children ≤16 years with an established diagnosis ‘ in 2023 were included. Primary outcomes were TT placement and audiometry, and difference in pre and postoperative hearing, expressed in Pur e Tone Average ( Results: A total of 482 children were included. TT’s we re placed in 71.2% (n= of which 34.4% (n= also had audiometric evaluation. Pre and postoperative audiometry was performed in 10.5% (n= In the 28.8% pa tients without TT’s (n= 58. 8% (n= had audiometric evaluation, significantly more often than in the TT group. A signficant improvement in PTA was found after TT placeme nt: median 24.4dB for the worse (n= and 14.4dB (n= for the better ear, measured a median of 6 3 days after TT placement. Conclusion: There is a discrepancy between the recommendations in the Dutch Guideline and current practice in surgical treatment of children with OM E audiometry is performed in the minority of cases When hearing evaluation was p erformed, a significant improvement on hearing was found after TT placement.
