Abstract

Title: The Interpretation and Impact of the Social Contract within the Samen Gezond IJssel-Vecht Regional Plan: A Qualitative Study on Policy and Practice Introduction: The Dutch healthcare system is under pressure. To continue ensuring accessibility, affordability, and quality, the Integraal Zorgakkoord (IZA) was established. Its regional implementation for this area is the Samen Gezond IJssel-Vecht regional plan. This plan describes that participation in the network is based on a social contract. A concept from political philosophy concerning the relationship between the state and its citizens, now applied to this large, diverse network. The conceptual nature of this contract raises questions about its various interpretations and their impact on collaboration and governance. Method: In this qualitative study, semi-structured interviews were conducted, transcribed, coded, and analyzed using ATLAS.TI. The 27 respondents included executives and alderman included in the Samen Gezond IJssel-Vecht network. Results: Interpretations vary widely. The contract is, among others, seen as a shared responsibility for care, a commitment to the regional plan, a guide for collaboration, and a relationship with residents. The value attached to the contract also varies significantly. There is a need for more concretization and clear guidelines. Collaboration within the network is seen as crucial but challenging. There is a need for open communication and discussing difficult topics. Regarding governance, there is a need for formalization and increased monitoring. Conclusion: Even though there is a broad moral commitment, the differences in interpretation create a need for clearer frameworks. The advice, therefore, is to work towards the concretization and formalization of agreements, along with stricter monitoring of progress. The consulted literature also indicates that this increases the chances of a successful collaboration.
