Introduction: One of the most common hereditary forms of Chronic Kidney Disease is Autosomal Dominant Polycystic Kidney Disease (ADPKD), characterized by cyst development in the kidneys. It is often diagnosed during reproductive years and can impact family planning decisions due to its genetic inheritance pattern, reduced male fertility, and potential adverse pregnancy outcomes in females. Understanding its influence on reproductive decision-making is crucial for improved preconception counseling and patient-centered care. This study aims to investigate ADPKD's influence on family planning and the most significant influencing factors. Methods: This retrospective quantitative cohort study included patients with ADPKD from four University Medical Centers in the Netherlands. Participants completed questionnaires regarding reproductive decision-making, fertility, pregnancy complications, and the openness to discuss ADPKD within households. Clinical data was collected at the study baseline, and multivariable regression analyses were conducted. Results: We included 693 patients of which 33.8% remained childless. We found that 13.8% of the childless individuals in our cohort was involuntary childless. 8.6% did not reach their desired family size due to ADPKD. Patients that were more likely to be affected by ADPKD in their family planning were female, younger at time of diagnosis, had a lower eGFR, had less favorable DNA mutation and less favorable Mayo class. Conclusions: ADPKD does impact the number of children and the ability to have children in a considerable percentage of the affected population. The understanding of influencing factors described in this study might help improve future preconception counseling of patients with ADPKD.
