Cannabis use occurs above average in psychosis-prone patients and is a risk factor for developing psychosis and it worsens the course of psychosis. It is therefore useful to understand why cannabis use is higher in this group than in people without psychosis susceptibility. In addition to the causal relationship between psychosis and cannabis use, a different cannabis perception between the groups or additional addiction may play a role. Cannabis perception of psychosis-prone users has rarely been completely investigated, reasons for not using cannabis and negative effects of use are lacking. This study has therefore broadly investigated the mental, social, and emotional perception of 24 psychosis-prone cannabis users compared to 33 cannabis users who were not psychosis-prone. For this purpose, a questionnaire was administered that accurately inquired about the tetrahydrocannabinol (THC) consumed for the first time. The psychosis-prone group used more than three times as much THC as the control group, although this difference was not significant. Risk assessment was comparable between both groups. The same applies for reasons for using or not using and the effects of cannabis. The psychosis-prone group did have a higher risk of cannabis addiction, although this did not have a significant association with cannabis perception. In conclusion, cannabis perception appears to be similar between those who are susceptible to psychosis and those who are not, so the treatment to reduce use may be the same regardless of the presence of psychosis susceptibility. However, screening for cannabis addiction in the psychosis-prone group is important, because of the higher risk of addiction.
