Abstract

Objective: The aim of this study was to gain insight into the nature and incidence of complications during or shortly after intubation in the Emergency Department (ED) of Medical Center Leeuwarden (MCL) and to identify potential associated factors with the occurrence of complications. Methods: This retrospective single-center cohort study examined complications that occurred during or after intubation of patients in the ED from January 1, 2019, to December 31, 2023. Information for each intubation was collected from the electronic patient record system, EPIC. The primary outcome was the nature and incidence of the observed complications. The secondary outcome involved investigating potential associated factors with the occurrence of complications. Results: A total of 353 patients were included in the study. Complications occurred in 56 (15.9%) patients, of which 26 were cases of hypotension, 16 of desaturation, and 9 of hypertension. Cardiac arrest was observed in 2 (0.6%) patients. Factors associated with the likelihood of complications included propofol as an induction agent, pulmonary and cardiac pathologies and multiple intubation attempts. Conclusions: The incidence of complications during intubation in the ED at MCL is consistent with international studies. The complications primarily include hypotension, desaturation and hypertension. The investigated risk factors allow for the identification of high-risk patients and the selection of optimal treatment strategies. These insights could help to minimize the incidence of complications in the future.
