Introduction: Anterior Cruciate Ligament (ACL) injuries are common in female athletes exposing them to adverse long term health outcomes. Our study investigates bone health among females who have undergone ACL reconstruction (ACLR). The objective is to examine interlimb asymmetries in Bone Mineral Density (BMD) and Trabecular Bone Score (TBS) at the knee. Also, we aim to explore the risk of developing Relative Energy Deficiency in Sport (REDs). Methods: In this case-control study, knee BMD was evaluated using dual-energy X-ray absorptiometry (DXA) with two distinct scan settings. Cumulative risk assessment scores for REDs were obtained through a survey. The exploratory part involved TBS assessment using DXA at the knee. The Wilcoxon signed-rank test was employed to ascertain significant differences between the ACL group and the control group. Results: Thirteen female student athletes with a history of ACLR were included in this study, as well as 13 controls matched for gender and sport. We found a significant difference in BMD at 15% of the femur [Z=-2.271, p = 0.023]. Also, 92.3 % of all student athlete participating in this study showed low risk scores for REDs. Lastly, TBS values were higher in the non-dominant leg. Conclusion: This study revealed metaphyseal bone loss, suggesting distinct remodeling processes at the metaphysis. Low REDs risk scores imply pre-existing poor bone health may not influence injury onset. Exploratory findings showed higher TBS values in the non-dominant leg.
