Maternal mortality remains one of the biggest global health concerns, as emphasised by Sustainable Development Goal (SDG) 3 aiming to reduce both maternal and neonatal mortality by 2030. Ethiopia faces challenges in meeting those goals. Empowerment is recognised as a tool to reduce maternal mortality. This study aims to identify the association of women’s empowerment (consisting of three domains; social independence, decision-making power and attitude towards violence) on adverse birth outcomes, complications during pregnancy and childbirth, antenatal care use and maternal health of women in the Tigray area in northern Ethiopia. Multivariable logistic regressions were performed with the four independent variables of women’s empowerment (total score and per domain) and the five outcome variables. They were adjusted for potential confounders. Significance was set at p <0.05. A positive significant association between women’s empowerment and maternal health was seen. Especially social independence (aOR 2.5, 95% CI [1.0-6.0]) and decision-making power (aOR 2.3, 95% CI [1.7-3.1]) increased the odds of a healthy mother. Moreover, a positive significant association was seen between women’s empowerment and adequate number of antenatal care visits, mainly driven by decision-making power (aOR 1.9, 95% CI [1.4-2.5]). However, there was no statistically significant association between women’s empowerment and adverse birth outcomes, complications during pregnancy and complications during childbirth. Empowering women in northern Ethiopia could have a positive effect on maternal health and adequate antenatal care. This may reduce the chance of maternal and neonatal mortality and therefore contributing to achieve SDG 3.
