Abstract

Background The presumed relation between dental disease and general health attracts increasing attention. The dental health status is related to the prevalence of dental decay and periodontal disease. Dental indices have been widely used to indicate the dental health status. The indices provide a quantitative method for measuring, scoring, and analysing dental conditions in individuals and groups. The most accepted and most frequently used indices in the literature to report dental caries and periodontal disease are the DMFT index and the CPITN index. In order to precisely determine a presumed relation between dental disease and general health it is important to know if the index used gives an accurate indication of the possibility to provoke a change in the general health condition. Research question The research question of this review was: ‘Are the indices used in dentistry effective in determining the microbial burden of dental disease during a certain time period and are they accurate enough to be used to determine a relation between dental disease and general health?’ Relatively little research has been published on this typical aspect of dental indices, and this justifies this literature research. Materials and Methods To obtain an answer to the research question, a systematic review was conducted. Appropriate articles were identified through searches in the databases MEDLINE (PubMed) and EMBASE. This by using relevant search terms, supplemented by searches of references within appropriate articles and key review articles. Results The combined electronic and hand-search resulted in 172 titles. After the selection procedure, seven articles were included. Two articles based on the CPITN index and five articles based on the DMFT index. The other studies were excluded because they did not fit the selection criteria. The articles of the DMFT index are based on its reliability and reproducibility comparing to new developed indices, for example the FS-T index or the T-health index. The studies about the CPITN index analysed if the CPITN index was a useful instrument for the assessment of the prevalence and severity of periodontal disease. Conclusion The CPITN index might not give a reliable representation of the oral health status because the results of the measurements with the ten index teeth difference significantly with the full-mouth-assessment. The results of a study can be biased because of this. A full mouth assessment should be used to describe the periodontal health. The DMFT index might not be useful because it does not measure the severity of the disease, or the bacterial load. The bacterial load is the condition of interest according to the relation with general health. The new developed index, the FS-T index, is the index of the first choice. It describes the amount of ‘functional tooth’. It gives a representation of the bacterial load, because if the score of the FS-T index is high, this gives an indication of the severity of the disease. Therefore this index should be considered to be used in the future to give a representation of the dental health in studies comparing the dental health with the general health.
