Abstract

Background: In the last decade, there have been numerous studies demonstrating the efficacy of SGTL2-inhibitors in treating heart failure, type 2 diabetes mellitus and chronic kidney failure. Following their implementation in the Netherlands, a protocol was established for the Isala Hospital region to monitor kidney function in patients with an initial eGFR <50 ml/min/1.73m2 after the initiation of SGLT2-inhibitors. The objective of this study was to assess the degree of adherence to this protocol by the medical doctors at the Isala hospital and to evaluate its relevance. Methods and results: CTcue was utilized to establish a database comprising all patients at the Isala Hospital who used an SGLT2-inhibitor from January 1st 2021 to September 1st 2023, with a recent eGFR not exceeding 6 months. This cohort encompassed 1418 patients, of whom 448 had an initial eGFR<50 ml/min/1.73m2. Within this subset, 285 patients (63,6%) underwent a kidney function assessment 1-5 weeks after initiating the SGLT2-inhibitor. Among them, 36 patients (12,6%) exhibited an acute eGFR decline of ≥25%. The median eGFR did not show a significant decline 1-5 weeks after initiating the SGLT2-inhibitor in the group with an initial eGFR<50 ml/min/1.73m2. Conclusion: Approximately one-third of the patients who, according to the protocol, should have undergone a kidney function assessment after initiating the SGLT2-inhibitor, did not receive one at the Isala Hospital. The number of patients with an eGFR decline of ≥25% shows the relevance of the current protocol, but there is room for improvements regarding implementation.
