Abstract

Background: Cerebral Palsy (CP) is a group of disorders in which spasticity in the lower extremities is a common problem and causes many disabilities. There are many therapies available in the treatment of CP, however some of those are invasive which causes pain and discomfort for the patient. It is therefore important to explore effective, less invasive therapies for spasticity. It seems that Extra Corporeal Shockwave Therapy (ESWT) appears to be one of those promising therapies. Methods: In this study we carried out an analysis of a retrospective cohort of 102 children with CP in which ESWT was applied in the routine care of spasticity in the lower extremities. The participants were treated in three different rehabilitation centers in the Netherlands, in Breda, Rotterdam and Beetsterzwaag. They received a series of treatments in which ESWT was applied three times during three weeks at the m. triceps surae (m. gastrocnemius and m. soleus), the hamstrings or adductor muscles. The primary outcome parameters we used were the (passive) range of motion (ROM) and for some of the participants the angle of catch (AOC) to measure spasticity. Measurements were taken before the start (T0), immediately after the third treatment (T1) and twelve weeks after the start of the treatment (T2). Results: A significant improvement was found in the ROM of the m. triceps surae (m. gastrocnemius and m. soleus) at T1 (respectively p <0.001 and p < 0.001), this significant effect persisted until T2 (respectively p < 0.001 and p < 0.001). A significant improvement was found for the hamstrings at T1 (p<0.001), which was further improved at T2 (p<0.001). Conclusion: This study showed a significant improvement in the ROM and the AOC in the lower extremities of children with CP after a series of treatment with ESWT. These positive results seem to persist over longterm. However, these results should be interpreted with caution due to the lack of a control group.
