Abstract

Objectives The daily medical handover is an important part of medical practice at hospital departments. Research has shown that a proper handover practice improves patient safety and contributes to better medical care. In the Dutch postgraduate medical education setting, the daily medical handover is compulsory for teaching hospitals. Therefore next to medical care, it plays a role in the education of medical students and residents. To date, there is little research on the educational value of medical handovers and on factors that might influence it. As a first step in the development of an instrument to score the educational value of medical handovers, this study explored medical clerks’ views on the educational value of medical handovers. Methods This qualitative research took place within two Dutch teaching hospitals. During five focus group interviews, 37 medical students were interviewed about their experiences with daily medical handovers. Because of their monthly switch between departments they have a unique position within these hospitals to compare the different handovers and reflect on the factors that increase or decrease the educational value of these gatherings. Focus group interviews were audio recorded and transcribed verbatim. The transcripts were analysed using thematic analysis. Results All participating clerks enthusiastically talked about their experiences with medical handovers. They discussed many factors that can influence the educational value of daily handovers. These factors can be complex, like attitude and working relationships between staff members, but also fairly simple like a clear structure of the handover and a proper introduction to clerks of what is being expected from them during handovers. This research also gives examples of the creative solutions that departments use to ensure that their handovers are as educationally valuable as possible. Conclusions The daily medical handover serves an educational purpose for multiple groups of students within teaching hospitals. This research gives insight into the factors that clerks perceive as supporting or hindering the educational value of these handovers. We advise hospital departments to evaluate the goals and set-up of their daily handovers, to improve them if possible, following the recommendations of our study participants.
