Abstract

Background: Eosinophilic Esophagitis (EoE) is a chronic immune-mediated disease of increasing recognition and prevalence. However, epidemiological population based data are scarce and often limited to referral hospitals and specialized EoE centers. The aim of this study was to provide accurate incidence rates from the total population of the Netherlands over the last 20 years. Materials & methods: For this follow-up study we used the nation-wide registry of pathology reports (PALGA) to conduct all reports of “esophageal eosinophilia” from 2011- 2015. We identified all EoE cases based on the diagnosis of the pathologist, > 15 eosinophils / HPF and degree of eosinophilia. Using the census annual population data incidence and prevalence rates of EoE in the Netherlands were estimated. Results: Between 2011-2015 a total of 2240 pathology reports matched the selected searching terms for ‘esophageal eosinophilia’ of which 1487 patients were classified as newly diagnosed EoE. The incidence of EoE increased from 0.01 per 100.000 persons in 1996 to 1.3 per 100.000 persons in 2011 and 2.1 per 100.000 persons in 2015. The current prevalence in the Netherlands is estimated up to 12.8 per 100.000 persons in 2015. We confirmed the welldescribed male predominance with a male-female ratio of 3:1 and average age for subjects with EoE from 30-49 years. No seasonal variation was found in the diagnosis of EoE. Conclusion: We calculated a rapidly rising incidence over the past 20 years for both children and adults in the Netherlands, therefore being the study with the largest timeframe to date. This emerging incidence of EoE has still not reached a plateau yet and signifies a high burden of disease.
