Abstract

BACKGROUND: Literature about the incidence, clinical course and psychological burden of COVID-19 in patients with a cutaneous immune-mediated disease treated with systemic immunosuppressive, immunomodulatory or topical treatment is scarce. OBJECTIVES: The aim of this study is to investigate the incidence and clinical course of COVID-19 related symptoms and the effect of the COVID-19 pandemic on clinical and psychological symptoms in patients with atopic dermatitis (AD) and psoriasis vulgaris (PsO). METHODS: We conducted a questionnaire-based cross-sectional study at the department of Dermatology of the Erasmus MC University Medical Center (Rotterdam, The Netherlands) between May 28th 2020 and June 23th 2020. RESULTS: In total, 347 AD patients and 264 PsO patients completed the questionnaire (response rate: 54%). The majority (n=339; 55%) were treated with systemic treatment. Forty-four percent of all respondents experienced at least 1 symptom that might be suggestive of COVID-19. There was no significant difference in the number and nature of COVID-19 related symptoms and cutaneous clinical condition between respondents with or without systemic treatment. Three respondents (0.5%) tested positive for COVID-19, resulting in hospitalization in 1 respondent. Twenty-nine percent of our respondents experienced a feeling of vulnerability in times of COVID-19 pandemic, and 74% reported an increase in their level of stress/anxiety. Respondents with systemic treatment were found to feel more vulnerable compared to respondents without systemic treatment (P <0.01). CONCLUSION: No significant differences were found in the incidence and clinical course of COVID-19 related symptoms between different treatment groups. The psychological burden of the COVID-19 pandemic might however be increased in patients with systemic treatment.
