Abstract

Introduction Pelvic organ prolapse (POP) is a common problem, however lacking an evidence-based first line treatment. A decision aid (DA) was developed to support decision making. The aim of this study is to analyse the effects of the DA on treatment decision, SDM and satisfaction with care, taking education level into account as a potential moderator. Methods In this cluster Randomised Controlled Trial (cRCT), 111 patients were included from five different hospitals: 40 in the intervention arm and 71 in the control arm. Participants filled out the online questionnaires on treatment decision, shared decision making (SDM-Q-9) and satisfaction with care (PSQ-18). Questionnaire data were coupled to baseline data and analysed in SPSS by Chi square test, Fisher’s exact, independent samples T test, Mann-Whitney U and logistic and linear regression for education level. Results No significant differences were found. A two-fold effect occurs when dissecting the study population based on level of education. Patients with a lower level of education are more likely to benefit from the DA by making a more conservative treatment choice, whereas patients with a higher educational status are more likely to pursue a surgical treatment option after using the DA. Patients with a lower level of education in the intervention arm report lower levels of SDM than those receiving standard care. Conversely, patients with higher educational status report higher levels of SDM when using the DA. Satisfaction was similar in all groups. Conclusion We demonstrated a differential effect of education level on treatment decision and SDM. Implications for clinical practice include keeping the education level of your patient in mind when implementing a DA.
