Abstract

Objective The aim of this study was to determine the change in success rate of chronic total occlusion (CTO) percutaneous coronary intervention (PCI) over time and to identify any patient, CTO or procedural predictors of technical success for a CTO PCI. Methods A retrospective cohort study was undertaken. Patients with a CTO PCI performed between July 2009 and December 2019 were included. The primary outcome was technical success of the procedure compared between 2009-2015 and 2016-2019. Results 386 patients were included. The success rate of a first attempt was 75,2% in the 2016- 2019 cohort. This is a significant increase compared to the 2009-2015 cohort (75,2% vs. 55,4%, p<0,001). No difference was found in the success rate after using a retrograde approach. After multivariate analysis hypercholesterolemia and a higher J-CTO score were found to be negative predictors of success of the CTO PCI. The odds to success were, corrected for these parameters, 2,334 times higher for patients in the 2016-2019 cohort compared to patients of the 2009-2015 cohort ((95%CI 1,317-4,136, p=0,004). Conclusions The success rates of CTO PCI have increased over the past 10 years. Some patients characteristics were associated with a lower chance of success of the procedure. The JCTO score was found to be a good predictor of success of a CTO PCI.
