Aim The aim of this study is to determine whether cardiopulmonary exercise testing (CPET) can predict perioperative complications in patients scheduled for Hepato-Pancreato-Biliary surgery (HPB). Methods In this retrospective study, high risk patients were included who had undergone CPET prior to liver, pancreas or biliary surgery. CPET variables, perioperative morbidity and organ specific postoperative complications were used as outcome measures. Results A total of 62 patients were included. Thirty-seven (60%) had postoperative complications. No CPET variable could significantly predict overall postoperative complications. The minute ventilation/carbon dioxide output ratio (VE/VCO2) at Ventilatory anaerobic threshold (VAT) was the only CPET variable that could predict organ specific complications. VE/VCO2 could predict pulmonary (P = 0.035), hematological (P = 0.049) and wound (P = 0.029) complications. Conclusions CPET variables could not predict overall postoperative complications after HPB surgery. VE/VCO2 may be useful for predicting organ specific postoperative complications.
