Background: Miscarriage is a common complication of pregnancy. In the first trimester, 10- 15% of the pregnancies leed into a miscarriage. A substantial part of the patients (50-85%) will get a spontaneous abortion within two weeks. If this does not occur or patients do not wish to wait, they will undergo treatment with misoprostol or a surgical evacuation. Studies who investigated these treatment options presented different results. Methods: In this retrospective study, the effect of misoprostol has been compared to dilatation and curettage by using data from the patient files. Also, the effect of a second course has been studied. A primary outcome was; the success rate of both methods. Secondary outcomes were the number of complications, the number of contacts after treatment and the time between treatment and follow-up. Results: In total, 438 patients were included. The mean age was 31.4 years and the gestational age was 10+1 weeks. The success rate after misoprostol was 58.7% compared to 87.7% after curettage. After the second-course misoprostol, 53.3% was successful and after curettage as a second treatment, this was 98.7%. More complications occurred after curettage. The number of contacts was higher with misoprostol treatment. Patients with a complete miscarriage after treatment returned to the hospital after a mean of 14 days. If a second course was required, patients returned after 10.9 days, 25.3% came back within a week. Conclusions: Misoprostol was successful in the majority of patients and less complications were found compared to curettage. A second course of misoprostol was also successful in a small majority, but further research is necessary to strengthen this evidence.
