Abstract

Introduction. A geriatric consultation with a comprehensive geriatric assessment (CGA) is recommended for older people with a positive score on the screening for frailty according to the safety management system (VMS). The medical center Leeuwarden (MCL) made a choice to optimize the basic level of care for frail elderly throughout the hospital. The aim of this retrospective qualitative study is to evaluate the current quality of care for the frail elderly patient in the MCL. Materials and methods. Twenty four patients ≥ 70 who were admitted to the emergency department between December 1, 2019 and February 29, 2020 and scored positive on the screening for frailty were included. The quality of care was analyzed by file research and patient survey by telephone. In order to gain more insight into the reasons why measures were or were not taken, the nurse specialized in geriatric patients from 3 wards were interviewed. The patients were interviewed by telephone regarding the information given regarding frailty during admission. Results. The aim of the MCL is that preventive and / or treatment measures are used in ≥ 90% of the patients with a positive VMS score. This was not achieved on any of the components. Problems experienced in compliance with protocols include changes in team composition, lack of time, financial resources, and transfer of knowledge. A large proportion of the patients (66.7%) were unaware of his / her frailty. Discussion. Based on this study, the conclusion can carefully be made that the "geriatization" of the MCL is not yet sufficiently successful. More awareness, training and cooperation will be needed in the future to achieve the desired basic level of care for frail elderly.
