Abstract

Background: Individuals with a psychotic disorder often have deficits in social cognition, as a result of which they have problems in social functioning. Deficits in the cognitive domain ‘Theory of Mind’ (ToM) are a strong predictor of social dysfunction. Social Cognition Training (SCT) improves cognitive processes, even though integration with daily life might be difficult. In this study SCT was extended with Virtual Reality (VR), an interactive and controllable method to practice in daily life situations. We will test the treatment effect of VR SCT on ToM and explore some moderators which might influence this treatment effect. Method: Sixty people with a psychotic disorder and lack of social cognition were randomized to the experimental DiSCoVR-condition (=28) or active control VRelax-condition (n=32). In DiSCoVR VR SCT attempts to improve social cognition, while people in VRelax learn to identify their own stressors, develop coping skills, receive psycho-education and explore relaxing virtual surroundings. Survey measurements will be performed at baseline and directly after a period of eight weeks containing sixteen individual (DiSCoVR or VRelax) sessions. The primary outcome is ToM, measured with The Awareness of Social Inference Test (TASIT). Paired- and independent sample t-tests will be performed to analyze the treatment effect. Covariates and moderators will be analyzed by regression analysis and ANCOVA. Results: No significant difference was found in the difference score on ToM before and after participation in the DiSCoVR-condition. The outcome on ToM was, regardless of the condition, influenced by the gender, duration of illness and level of education of the participants. Conclusion: In contrast to previous research, VR SCT did not improve ToM in this study. Nor were any moderators found to influence the relationship between VR SCT and ToM.
