The aim is to get insight in the patient’s pathway of men with chronic urinary retention (CUR) in the hospital, by making an observation of the different treatment options, the related complications and the consequences of these complications. To achieve this, male non-neurogenic patients with a post-void residual >150 ml existing for at least four months treated by an urologist in 2014 were retrospectively analyzed. All events from the fist treatment until September 2020 were recorded and divided in treatment options, complications and consequences of complications e.g. type of hospital contact, additional diagnostics and new treatments. Different treatment options were clean intermittent catheterization, urethral catheter (UC), suprapubic catheter, desobstructive prostate surgery and watchful waiting (WW). In this study, 177 patients could be included with a median follow-up of 68 months (range 1-139). Almost all patients started with a form of catheterization (98%) and most of them also ended with it (85%). During the follow-up period 28% (n=49) had desobstructive surgery, giving them more chance of ending up in the WW-group. By far the most common complications was a urinary tract infection (n=685), followed by macroscopic hematuria (n=71), catheter problems (n=68), catheter related pain (n=56) and post renal problems (n=40). The incidence rates of all complications were highest in patients with a UC, except for post renal problems that occurred most frequent in the WW-group. In conclusion, most male patients with CUR are treated with catheterization. UC has the highest chance of complications, except for post renal problems. Selected patients could benefit from desobstructive surgery.
