Abstract

Background: neonatal hypoglycaemia (NH) is a common problem in newborns resulting in permanent neurological damage if insufficiently recognized or treated. The use of oral glucose for the prevention and treatment of NH is relatively new and is rarely used. Its use leads to a decrease in intravenous glucose administration which seperates mother and child. Since April 2019, a new NH-protocol has been implemented at Isala Hospital in which oral glucose administration is used. In addition an absolute weight limit of 2500 grams has been implemented instead of a birth weight <P10, for control of bloodglucoselevels. Aim: to evaluate the effectiveness and safety of the new NH-protocol, specifically the application of oral glucose administration and the absolute weight limit of 2500 grams. Method: it concerns a retrospective care evaluation study in the Isala Hospital between 1 November 2018 and 31 December 2019. Neonates screened according to the new NH-protocol are retrospective compared to the group of neonates screened according to the old NH-protocol. The effectiveness was analyzed on the basis of intravenous glucose administrations, the first and lowest glucose values and the type of food, formula- of breastfeeding. The safety was assessed by looking at the complications related to oral glucose administrations. Results: implementation of the new NH-protocol resulted in a significant decrease in NH intravenous glucose administrations (10.8% versus 5.9%). On average, oral glucose was applied 1.6 times, with mild complications occurring in 3.5%, all without clinical consequence. Conclusion: the implementation of the new NH-protocol with oral glucose administration reduces the number of intravenous glucose administrations by half and is safe to use.
