Abstract

Introduction: Studies show that up to 50% of the children with asthma suffer from poor or uncontrolled asthma, despite optimal treatment, which could be a reflection of their therapy adherence or inhalation technique instead of refractory disease. However, little is known about the relationship between therapy adherence and inhaler technique and patient-related factors, such as age, gender, parental education, living environment and hospital rates. The goal of this study is to gain insight into this relation and to identify relevant patient-related factors. Methods: Children with uncontrolled asthma, included from October 2019 through May 2022 at Medisch Spectrum Twente (MST), were observed during a four-week period using the Respiro™ add-on device with their Formoterol/beclomethasone Nexthaler. This device measured adherence patterns and inhalation errors through analysis of vibration patterns, associated with inhaler use. Demographic data were gathered from the electronic health record of MST. Therapy adherence and inhalation technique were assessed as both continuous and categorical variables. Results: 25 patients were included. There was a non-significant, weak to moderate correlation between inhalation technique and therapy adherence, with a correlation coefficient of 0.375 (p=0.06). Univariate analysis showed a significant association between adequate adherence to therapy and the number of emergency room (ER) visits (p=0.031). In univariate analysis, also a significant association was found between poor inhalation technique and height (p=0.032), educational level of parents (p=0.031), the number of ER visits (p=0.014), baseline lung function reversibility (p=0.046), and baseline (Childhood) Asthma Control Test-score (p=0.046). Conclusion: To conclude, there was a non-significant, weak to moderate correlation between therapy adherence and inhalation technique in this study. Furthermore, several patient-related factors were identified with a significant relation to these two aspects of inhaled medication use. However, since this thesis is an interim analysis with a small sample size, further analysis after completion of inclusion is recommended.
