Abstract

Background: Weight loss after bariatric surgery is often one of the most important outcomes and used to determine whether the surgery has been a success. The Roux-en-Y Gastric Bypass (RYGB) and the Mini Gastric Bypass (MBG) have both proven to result in substantial weight loss. However, comparative long-term data on these procedures are lacking. This study focusses on differences in weight loss outcomes of RYGB and MGB over a follow-up period of five years. Methods: This single-centre retrospective cohort study included patients who underwent a primary RYGB and MGB between January and December 2016 in the Centre Obesity North-Netherlands at Medical Centre Leeuwarden. Primary outcomes were percentage Total Weight Loss (%TWL) and Textbook Outcome Weight Loss (TOWL) after five years of surgery. Secondary outcomes were absolute weight, Body Mass Index (BMI) and percentage Excess Weight Loss (%EWL). TOWL was used to assess ideal weight loss after surgery, which was defined as %TWL between 20 and 35%. Results: 558 patients (103 men and 455 women) underwent bariatric surgery, including 124 patients for RYGB and 434 patients for MGB. No significant differences were observed between both procedures in %TWL, %EWL and TOWL achievement after five years of surgery (%TWL 24.5±9.91 vs. 27.0±10.18, p=0.066, %EWL 63.6±25.86 vs. 69.3±26.8, p=0.107, and TOWL 50.7% vs. 55.5%, p=0.102). However, MGB resulted in significant better weight loss outcomes up to four years after surgery (%TWL p=0.003, %EWL p=0.003, and TOWL, p=0.024). Conclusion: This study has shown that RYGB and MGB achieved similar weight loss outcomes in the long-term. However, MGB resulted in better weight loss outcomes in the short- and mid-term. Therefore, regarding weight loss in the long-term, RYGB and MGB could both be advised to patients.
