Abstract

Introduction The vital parameters of premature infants are being monitored to search for incidents (apneas, bradycardias and desaturations). So far, a nationwide protocol for the monitoring of late premature infants doesn’t exist, leaving each hospital to create their own. This has caused a huge variety between protocols. This prospective study evaluates the monitored incidents of late premature infants of 34 to 36 weeks. In addition, the opinions of the nurses, pediatricians and nurse specialist, who are working at Isala Zwolle, were took into account. Methods. From October 2021 to February 2022, the amount and type of incidents were tracked for late premature infants of 34+0 to 36+0 weeks for the first two to ten days postpartum. In addition, depth-interviews were conducted with nurses, pediatricians and nurse specialists. Results 21 Children were included and 61.9% of them showed incidents. Relevant incidents (the need to be tapped or receiving additional oxygen) were found in 23.1% of the incidents. Three children needed CPAP or caffeine. Infants of 34 weeks showed a peak in the number of incidents at day four, while infants of 35 weeks showed no evident peak. When shortening the monitoring duration for infants of 34 weeks to 72 hours with the last 48 hours without incidents, relevant incidents will be missed in one child. When only monitoring infants of 35 weeks on indication, for example when infection is suspected, relevant incidents will be missed in one child. The in depth-interviews revealed that a good implementation, for example a clinical reasoning lecture, would facilitate getting used to a new protocol. Conclusion Monitoring for at least 72 hours with the last 48 hours without incidents would be a suitable duration for late premature infants of 34 weeks. Monitoring on indication is suitable for late premature infants of 35 weeks.
