Abstract

Purpose: A patient’s experience of pain and disability in the upper limb is influenced by their mindset and circumstances. Symptoms of depression correlate with pain intensity and magnitude of disability in patients with upper extremity illness. Some patients have diagnosed and treated depression with limited current symptoms of depression. Others have substantial depression that is often undiagnosed or undertreated. The aim of this study was to determine the prevalence of an estimated diagnosis of major depression in an orthopedic hand and upper extremity population and factors associated with it. Methods: In this observational cross-sectional study, a total of 190 patients completed questionnaires measuring depression with the Patient Health Questionnaire (PHQ-9), upper extremity disability with the Patient-Reported Outcomes Measurement Information System (PROMIS) Upper Extremity CAT, pain interference with the PROMIS Pain Interference CAT, and health anxiety with the Short Health Anxiety Inventory (SHAI-5). Results: The prevalence of an estimated diagnosis of major depression (PHQ-9 ≥ 10) among orthopedic hand and upper extremity patients was 12% in our sample. PROMIS Pain Interference (OR = 1.2; 95% CI 1.0-1.3; P = 0.005), having multiple pain condition (OR = 5.5; 95% CI 1.7-18; P = 0.005), and a history of depression (OR = 4.1; 95% CI 1.4-12; P = 0.011) are independently associated with a PHQ-9 score of ≥10. Conclusions: Major depression in an orthopedic hand and upper extremity population seems to be associated with the impact of pain on the patient’s life, not with the severity of disability of the upper limb.
