Abstract

Background: Benign prostatic hyperplasia (BPH) is one of the most common non-malignant neoplasia in men. Traditionally, open surgery is the most customary treatment for large prostates. Despite positive results it still remains an invasive operation with complications. Therefore, multiple minimal invasive interventions have been developed to achieve an equal effectiveness with less morbidity and a faster recovery. The aim of this study is to compare short- and medium-term effectiveness and peri- and postoperative complications between the open enucleation and the transurethral bipolar enucleation in prostates larger than 80 mL. Methods: A retrospective cohort study was executed in ZGT between October 2021 and April 2022. Men with prostates above 80 mL were included. Primary outcome measures are: IPSS, quality of life score, maximum flow rate, urine residue after micturition and nocturnal micturition frequency. Secondary outcome measures include removed prostate tissue, operation time, hospitalisation length, blood loss, blood transfusion, clot retention, UTI, incontinence and reinterventions. Results: Both IPSS and quality of life score were statistical significant better in the open group. Further functional outcomes were statistically significantly similar. The open enucleation has a shorter operation duration with more tissue removal, the transurethral enucleation has less blood loss and a shorter hospitalisation length, all statistically significantly different. The perioperative complication rate was not statistically significantly different. Noticeable, incontinence 3 months after discharge was statistically significantly higher in the transurethral enucleation. Conclusion: The open enucleation is an effective intervention with favourable functional outcomes. The bipolar enucleation has potency to be a less invasive alternative for BPH with large prostates, however due to the high percentage of incontinence, this procedure is not preferred at the moment and more scientific research is necessary.
