Introduction: Morphodysphoria is profound dissatisfaction with one or more aspects of the own body. Its pathological form is the ‘bodydysmorphic disorder’ (BDD), which causes significant suffering. This involves a disturbed body image; the patients experience certain body parts as very ugly, while this is hardly or not at all visible to others. Due to the growing dissatisfaction of people with their own body in today's society, doctors and oral care professionals see patients with morphodysphoric characteristics more often. It is important to recognize morphodysphoria and morphodysphoric disorder, because, especially for the latter, physical treatment will only lead to further dissatisfaction. A screening instrument can help to recognize this in time. The aim of this study is to develop and test the usefulness of a screening instrument for morphodysphoria in the population of facial surgery patients of the department of oral- and maxillofacial surgery (‘MKA’ in Dutch) at the University Medical Center Groningen. The nature and severity of morphodysphoria and the prevalence of morphodysphoric disorder have also been studied. Material and method: The study enrolled 27 patients (N= 27) who had registered for facial surgery in the MKA department. Two questionnaires were used for this study: the BDDQ-MKA (for the patient) and the evaluation list (for the MKA surgeon). The patient questionnaire is a composite of the Body Dysmorphic Disorder Questionnaire (BDDQ) - a validated screening tool for BDD – complemented by questions based on the definition of BDD in the DSM-5, and adapted to MKA surgery. The evaluation questionnaire was based on an existing questionnaire and adapted for this target group. Results: The BDDQ-MKA questionnaire was especially helpful in determining morphodysphoria in patients who had complaints other than physical symptoms, and in gaining insight into the nature and severity of the suffering from patients with a relatively high total score. Although most patients reported no or low morphodysphoric symptoms, the prevalence of the morphodysphoric disorder in the study population was 28%. There was a negative correlation between the severity of the morphodysphoric characteristics and age, but no difference between men or women or type of intervention. The sensitivity of the BDDQ-MKA was 20%, the specificity 100%. Discussion: Although the BDDQ-MKA requires further research and optimization (eg increasing the sensitivity), it is perceived as clinically useful in patients who wish to undergo facial surgery for non-physical complaints. It gives the practitioner insight into the nature and severity of morphodysphoric complaints, which is very valuable in providing the right care for the patient. However, additional psychodiagnostics remain impoortant. The high prevalence of the morphodysphoric disorder in this study and the severity of the morphodysphoric features show that morphodysphoria is a problem that should not be underestimated in facial surgery.
