Abstract

Objectives. Orthodontically induced external apical root resorption is one of the most seen unwanted side effects during an orthodontic treatment. There is some uncertainty among orthodontists on how to respond to clinically relevant external root resorption. Therefore, a clinical practice guideline (CPG) was developed by the Dutch Association of Orthodontists together with the Knowledge Institute of Medical Specialists (KIMS). This CPG provides the current knowledge about root resorption and clinical recommendations. The aim of this study was to evaluate the effectiveness of the CPG on the response to root resorption by Dutch orthodontists after publication in 2018. Materials and methods. A digital questionnaire was sent to all 275 active working orthodontists in the Netherlands. The first part of the questionnaire consisted questions about the clinical practice with regarding tot the 13 recommendations before (t0) and after (t1) publication of the CPG in 2018. The recommendations were divided into 4 categories; diagnosis (2), risk factors (2), treatment strategy if root resorption is diagnosed (6) and after-treatment care requirements (3). Background information of the individual orthodontists was assessed by 4 questions in the second part of the questionnaire. The degree of following the recommendation was measured by a 7-point Likert scale with the following response options; ‘never, very rarely, rarely, neutral, frequently, very frequently and always’, which were linked to the numbers 1 to 7. Results are shown in a frequency table. The effectiveness of the CPG was tested by the Wilcoxon test. The Mann-Whitney U test and a backward prediction model was used to analyze the influence of the demographic variables. Results. Questionnaires were completed by 132 orthodontists (response rate of 48%). 55% were male, 81% had been trained in the Netherlands, 89% had ≥ 6 years of experience and 89% worked in a private orthodontic office 130 orthodontists (98,5%) showed a positive change in clinical practice after publication of the CPG in 2018. The biggest change in behavior with regarding to root resorption was seen in the part Treatment strategy if root resorption is diagnosed. The demographic variables sex, clinical experience, country of study and working intramural or in a private orthodontic office had no significant effect on following the recommendation of the CPG. Conclusion. Implementation of the CPG had a positive effect on following the recommendations about root resorption by orthodontists in the Netherlands. This contributes by achieving a more standardized way of practice for the profession and reducing variation between practices.
