Abstract

Introduction: Conditions affecting oral health such as caries and tooth wear can have a negative effect on children’s Oral Health-related Quality of Life (OHRQoL). The OHRQoL is measured by the Child Oral Health Impact Profile (COHIP), which assesses the impact of oral diseases on quality of life in children. According to ‘2018 Signalement Mondzorg’, an increase in caries prevalence and dental wear in 11- and 12-year-olds has been observed in recent years, particularly in children with a low social economic status (SES). It is unknown what effect this increase has on the OHRQoL. In addition, it is unknown to what extent children perceive the importance of certain oral health aspects. The purpose of this study is to evaluate the OHRQoL and oral health aspects in 11-and 12-year-olds and to determine the association with SES. The research questions are; ‘What is the difference in the OHRQoL overall and by COHIP-domain between a high and low SES group among 11- and 12-year-olds?’, ‘What is the difference in the perceived importance related to the individual and combined oral health aspects among 11- and 12-year-olds between the high and low SES group?’ and ‘What is the correlation between OHRQoL and the perceived importance related to the individual and combined oral health aspects among 11- and 12-year-olds?’. Methods: A cross-sectional study was conducted in children who attended the 7th and 8th grade in elementary school in Enschede and Groningen. The participants were invited to complete a paper survey on OHRQoL and the importance of oral health aspects. The survey also collected demographic data, such as the level of education of the child’s mother. This level of education was used as a proxy for the child’s SES. Results: A total of 97 children aged 11-12 years were included in this study. There were no statistically significant differences in OHRQoL, COHIP domains and the perceived level of importance of oral health aspects between children with a high (n= 48) and low (n= 49) SES group. Individual questions on importance of oral health aspects showed a statistically significant difference between the SES groups in 'protecting teeth from trauma’ (U = 902.500; z = -2.092; p = .036). Children with a high SES considered this aspect to be more important. In addition, positive significant correlations were found between OHRQoL and the importance of oral health aspects (individually and combined). Conclusion: In this study, no significant differences were found between children's SES in OHRQoL, COHIP domains and importance of oral health aspects combined. However, the SES does have a statistically significant effect on the individual oral health aspect; 'protect teeth from trauma'. A higher score on the perceived importance of the oral health aspects was associated with a higher importance of OHRQoL in children aged 11-12 years. These findings can be taken into account when developing prevention strategies aiming to improve OHRQoL. It is recommended that these strategies not only focus on solving oral health problems, but also take into account the value placed on importance of oral health aspects.
