Abstract

Background: When patients with coronavirus disease 2019 (COVID-19) on the Intensive Care Unit (ICU) were not clinically improving of clinically deteriorating, additional diagnostic testing, including bronchoscopy with broncho-alveolar lavage (BAL), was performed to rule out potentially treatable diseases such as for example bacterial and fungal superinfection. The main aim of this study was to investigate the therapeutic consequences of BAL. Our hypothesis was that BAL did not result in significant changes in therapeutic management. Methods: In a retrospective analysis of a cohort of patients with COVID-19 on the ICU of the St. Antonius Hospital in Nieuwegein, the Netherlands, we compared the administration of antibiotics, antifungal therapy and prednisolone on the day of BAL and three days after. Results: Of 73 patients with COVID-19 who underwent BAL, changes in medication regime were observed in 62% of patients. In 36% of patients antibiotic management changed, in 21% antifungal management and in 34% prednisolone management. Conclusions: In patients who underwent BAL there was a change in antibiotics, antifungal or prednisolone management in 62% of cases.
