Abstract

Obesity is a worldwide problem. It is known that people with BMI ≥ 25 have a higher risk for the metabolic syndrome. Studies show that not all people with BMI ≥ 25 experience this increased risk. Until now studies show no consistent results of the association between periodontitis and the metabolic syndrome. The objective of this study was to analyze the association between the periodontal status and the metabolic phenotype of people with BMI ≥ 25. Data were extracted from the National Health and Nutrition Examination Survey (NHANES) from 1988-1994 and 2001-2004. Only people with BMI ≥ 25 were included in this study. Based on their periodontal health people were divided into two groups: periodontal healthy or periodontal unhealthy. After that they were divided into a metabolic healthy group (MHO) and metabolic unhealthy group (MUO) based the IDF definition for the metabolic syndrome. This definition uses criteria for: blood pressure, triglyceride levels, HDL-C levels, and glucose levels. The association was tested using a multivariable binary logistic regression model. Confounding factors sex, age, ethnicity, smoking behavior, and CVD were added to the model. An interaction analysis was performed to analyze the influence of the factors sex, age, ethnicity, and smoking behavior on the association between periodontitis and the metabolic phenotype. These analyses were also performed after dividing people into an MHO and MUO group based on more stringent criteria and the ATP III-definition for the metabolic syndrome. This study finds an association between the periodontal status and the metabolic syndrome using the IDF-definition, more stringent criteria, and the ATP III-definition for defining the metabolic status. The association can also be found when confounding factors are added to the model. Results regarding the influence of factors as sex, age, ethnicity and smoking behavior show now consistent results when using different definitions for defining the metabolic status.
