Abstract

Since the introduction of ‘Invisalign’, which is a transparent orthodontic aligner, in society the demand of these type of braces tremendously increases. The current situation of the increasing use of Invisalign braces in adult patients in combination with inconclusive scientific conclusions makes it necessary to assess the predictability of Invisalign braces when expanding in the maxilla. Besides the predictability of expansion in the upper jaw, it is also relevant to evaluate the influence of factors as gender, age and treatment duration on this. The aim of this study is to gain knowledge about the predictability of a treatment with regard to expansion in the upper jaw using Invisalign braces. So that these braces can be used in more adult patients who do not want permanent orthodontic facilities due to an aesthetic wish. The research question is as follows: To what extent does the actual expansion correspond to the expected expansion at the height of the maxillary cuspids, with the use of Invisalign braces in adults? The study population was selected from the patient base of patients treated for maxillary cuspid expansion by the orthodontist, if the initial and final scans were available. In this study, the difference in size of the two expansions has been calculated, i.e. actual expansion minus the expected expansion. If this difference is greater than 0.3 mm, this study refers to an incorrect estimate. In total 91 patients were selected. In 44 of these patients the difference between the expected and the actual expansion is 0.3mm. This is a percentage of 48%, 95% CI [37.7;58.3]. In 47 patients the difference is > 0.3 mm. Within the limitations of this study, it can be concluded that Invisalign with Smarttrack material is not a predictable treatment option for maxillary cuspid expansion. The influences of gender, age and treatment duration do not influence the difference between the expected and actual expansion. Further research about expanding the maxilla with a distribution of data in equal groups seems justified. In the pilot study, seven patients were selected. The target of fifteen patients was not achieved due to the low availability of distalization cases, since no initial and final scan were present. The pilot study was small and the group with a difference in distalization of >0.3mm was larger compared to the group with a difference of less than 0.3mm. It is relevant to test the hypothesis again with the use of a larger population, and to investigate the influence of factors such as age and gender and a possible interaction between the two.
