Abstract

Background: Fractures of the femoral shaft mainly result from high energetic trauma’s. Surgical intervention is necessary in almost all cases. Despite constant improvement of operation techniques and materials, about 10 percent results in nonunion of the bone, with even higher rates in open fractures. This study focused on bone healing after pseudoartrosis treatment. Methods: A retrospective cohort study was performed within the University Medical Center Groningen (UMCG). Results of pseudoartrosis treatment performed at the department of trauma surgery between 2007 and 2022 were reviewed. Primary outcome measures were union rate and time to union. Individual treatment results were compared using a t-test. Results: 58 patients were included. Within primary pseudoartrosis treatment, 34 patients were treated using plate osteosynthesis where 21 were treated using nail exchange. Union rates were 70.6% and 61.9% respectively, whereas mean time to union was 17.4 and 17.6 months respectively. 1 patient was treated using plate augmentation, 2 were treated by intramedullary nail osteosynthesis. 17 patients underwent single or multiple revision surgery’s. In 6 patients union did not occur. Conclusion: Nail exchange proved not to superior over plate osteosynthesis to achieve union of the fracture. Also, use of dynamic condylar screw (DCS) and plate osteosynthesis were shown equally valuable as a treatment for pseudoartrosis of the proximal femur. Usage of spongiosa grafts in femoral pseudoartrosis treatment showed significant higher union rates. Plate augmentation shows to be a promising technique which should be further investigated
