Abstract

BACKGROUND As we age, the likelihood of developing disease increases. The more disease, physical and psychological discomfort one experiences, the more difficult it often is to take good care of one's mouth and visit the dentist, which can lead to a decline in oral health and related quality of life. To measure the oral health related quality of life, a questionnaire has been developed; the Oral Health Impact Profile (OHIP). This is a widely used questionnaire that is used in many studies for all age groups. The question is whether this questionnaire is suitable for all age groups, as not all age groups are equally affected by similar oral health problems. For example, oral health problems may be qualified very differently by young people than by older people because older people may have a different perception or frame of reference due to different life experiences. Therefore, the OHIP questionnaire is perhaps not equally suitable for all target groups or should be analyzed differently for each target group. The aim of this study is to find out whether the questionnaire is completed in the same way by younger and older people with similar dental restoration rate/oral health problems and whether this questionnaire is applicable to both generations. The question is: ‘Does the age of a research participant affect the outcome measure of the OHIP questionnaire in dental research?’. MATERIAL AND METHODS At random, elderly (75+) and young (20-30 years) patients in the CTM were approached for participation in this study. A total of 232 individuals were approached. All participants completed the OHIP questionnaire and the EQ-5D-5L (overall quality of life). The elderly were additionally asked to complete the GFI (Groningen Frailty Indicator) and IM-E-SA (Intermediate Elderly Self-Assessment) questionnaire to obtain an understanding of their health and functioning. The dental restoration rate/oral health was determined from the treatment journal using the ‘KNMT-abonnementstarieven’. Outcome measures were the effect of age and health (frailty) on the perception of oral health at similar dental restoration rates. RESULTS 84 adolescents and 80 elderly people were included in the study. The elderly in the study scored significantly lower on the OHIP questionnaire than the young participants (p=0.028). Oral health/restoration rate was found to have no significant effect on the OHIP score (p= 0.006). No effect of age on the relationship between oral health/restoration rate and OHIP score was found (p=0.000). No relationship between overall quality of life and mean OHIP score was found (p=.0141). The degree of frailty was found to have no significant effect on the OHIP score (p=0.142). There were significantly more older people who found the OHIP questionnaire inapplicable to their situation than younger people (p=<0.001). DISCUSSION The study used ‘KNMT-abonnementstarieven’ to determine oral health/restoration rates. As a result, all older participants with their own dentition where more restorative work has been done or had a full denture fell into the poor oral health/high restoration category. Previous research has used oral status (dentate, edentulous, denture on implants) to classify participants. The choice to classify dental restoration rate/oral health 4 through the ‘KNMT-abonnementstarieven’ may have painted a one-sided picture of the older group because they tend to have high restoration status compared to younger people. On the other hand, a one-sided picture is also created when relying on oral status, because there are almost no young people with a full denture. Many older people found the question of applicability complicated, and these outcomes should therefore be interpreted with some caution. CONCLUSIONS Age has a significant effect on the outcome measure of the OHIP questionnaire with similar oral health/restoration rate of the teeth. A high degree of restoration rate or poorer oral health of the teeth seems to have less impact on quality of life in the elderly than in the young people. Older people found the OHIP questionnaire less applicable to their situation. This makes the OHIP questionnaire less applicable for use in dental research among the elderly based on the results of this study.
