Background: Penile cancer is a rare disease. Surgical treatment has a huge impact on these patients in multiple ways. The aim of the current study was to evaluate health-related and sexual quality of life in patients treated for penile cancer. Material and methods: A retrospective evaluation of prospectively collected questionnaires was conducted for patients treated between 2016 and 2021 in a single high volume/reference center for penile cancer. The questionnaires contained questions from the EORTC-qlq-C30, IIEF-15, MSES, CWS and a custom section on micturition. Mixed effect models were used to test multiple differences over time. The patients were divided in two surgical groups: “organsparing” and “amputative”. A sub-analysis was performed for patients aged <70 and patients aged ≥70. Results: A total of 242 patients were analysed. The organ-sparing group scored significantly better on health-related quality of life and sexual functions compared to the amputative group, both pre- and post-operatively (p<0.001). Overall sexual satisfaction was found to have a significant impact on health-related quality of life. The <70 group scored significantly better on health-related quality of life and sexual functions (p<0.001) and improved significantly over time (p<0.001). Older patients (≥70) maintained their satisfaction over time. Conclusion: Organ-sparing surgery for penile cancer is associated with better outcomes on both health-related quality of life and sexual satisfaction. Although patients treated with more amputative surgery are often also able to maintain sexual satifaction, a more specific questionnaire for penile carcinoma should be developed in the future to provide more insight and enable more specific recommendations and patient counseling
